The hardware is relatively simple: it requires only power and a wi-fi connection.  Most of the heavy lifting is done securely in the cloud.  This means fewer things that can go wrong and force downtime, or require repairs.  And it means regulatory and compliance considerations are covered by your existing policies.

aRNi is a medical virtual assistant powered by a touchscreen monitor that can be placed on a wall or desktop. It works similarly to household digital assistants: just say “Hey aRNi,” followed by a question, and aRNi will provide answers and options via voice and a touchscreen.

aRNi draws its data directly from your organization’s trusted sources like Wolters-Kluwer Lippincott Procedures and Lippincott Advisor, and American Society of Health-System Pharmacists AHFS Clinical Drug Database, with more in development. This provides accurate and relevant information on pharmacology, treatments, complications, and side effects. The user can then ask follow-up questions and navigate menus by speaking or using the touchscreen.

Installed in proximity to patient care areas, aRNi allows practitioners to access, examine, and apply vital information right where it’s needed.

Why aRNi

The Problem
Preventable medical errors are a leading cause of injury and death in hospital settings, resulting in tens of thousands of lives lost every year. Clinicians are under ever-increasing demands to understand an ever-growing number of drugs and interactions, disease processes, and new technologies, and many medical errors are caused by lack of access to information at the point of care. In addition to the grave personal costs, a medical error costs a hospital over $50,000 on average per incident.

We believe these errors are preventable. Internet search engine usage is common among healthcare professionals, but general searches often give irrelevant results, conflicting information, and poorly prioritized sources. Many hospitals also spend millions of dollars annually for topic-specific content databases, but practitioners often can’t access them near the patient.

For clinicians to provide the most appropriate care, current evidence-based information, obtained via credible resources, must be easily accessed, assessed, and applied.
How It Compounded
These problems are worsened by the current COVID-19 pandemic. Many healthcare workers are being upskilled and retrained to work in areas of a hospital that are unfamiliar to them, especially clinicians in operating rooms, post-anesthesia, and ambulatory care, whose normal work has been placed on hold during the pandemic due to distancing and the need for beds for COVID-19 cases.

On one of our first tests in a clinical environment, we chose to put aRNi in a COVID-19 unit. We set the unit up in a med room where practitioners pull medications. A junior nurse was checking out a new COVID-19 medication that cost $8k per dose, and asked if it was compatible with another of the patient's medications. The Senior nurse didn't know — it's a brand new medication — and said "Why don't you ask aRNi?".

Within a few seconds, aRNi sorted and accessed the information and verbally gave a clear answer on drug interactions. That day we knew aRNi would save lives.

Has aRNi been tested by respected medical institutions?
  • aRNi has been tested and used by clinics professionals in a variety of clinics settings since 2020. Results are very encouraging, and at this point unpublished. Clinical trials are ongoing.
What got aRNi started?

A realization that a response to the workforce shortage, new graduate nurses were graduating from programs with significant knowledge gaps that were reaching the patient. The current fast pace and task focused environment did not allow time for clinicians to spend time to search for answers.

How can I trust the answers?
All answers come from pre determined data sets selected by the health system. aRNi also references what specific data source is used when giving an answer.
What are the regulatory and compliance concerns?
aRNi does not connect with the EHR or any patient data. aRNi uses data sets already in use and reflected in hospital policies.
What do I need to run aRNi?

About Creations 86

Our founder Brian is a chief nursing officer (CNO) who has held leadership roles at five respected hospital systems across the US.  "I refuse to go through the rest of my professional career living 1 shift (12 hours) at a time. Daily firefights to solve staffing are very time consuming and indicative of instability," he said.  "We are all familiar with the metrics that follow high turnover and unstaffed units." 

So how to solve it?  "Technology and innovation must be looked upon as possible solutions to this workforce shortage."  He recognized "Moore’s Law says that our technology capabilities will double every 2 years. How can we keep up with technology that is evolving this fast?"

Brian wanted to answer the question "How could technology enable nurses to access accurate information faster and more effectively?" In 2017 Brian worked with a team of nursing professionals and healthcare engineers and founded "a startup company that focused on developing technologies and processes to improve nursing workflows and efficiency."

"Since most of the information millennials get is from voice-activated searches such as Siri or Alexa, I wanted to focus my innovation on putting information into nurses’ hands using voice-controlled search technology," he said. And with a team of seasoned entrepreneurs, "we created a customizable, speech-enabled virtual assistant that allows clinicians to ask a wide variety of questions in the health care environment related to their patient’s care and receive answers not from the world wide web but from predetermined clinical data sets at the point of care."

With a starting grant from Indiana University School of Medicine, the newly-formed Creations86 team constructed a proof-of-concept hardware and developed software architecture leveraging the latest methods of machine learning and natural language processing techniques to give aRNi the latest in speech recognition, text processing, and classification.

aRNi has completed several rounds of trials with multiple hospital systems and continues ongoing phase testing with medical professionals in a variety of clinical settings. We're very encouraged by the results and excited to bring aRNi to healthcare providers to increase confidence and accuracy, improve efficiency and effectiveness, prevent medical mistakes, and to thereby bolster the mental well-being of our workforce.
